When you unload a security device, then the wrong device list entry gets removed. When re-opening the security devices dialog the error disappears.
Reproducible: Always
Steps to Reproduce:
1. Edit -> Preferences -> Advanced -> Encryption -> Security Devices -> Load
2. select some working PKCS#11 device
3. Security Devices -> Unload
4. Check out the security devices list

I can't reproduce this. I assume it's been fixed in the past 8 years, but feel free to reopen if you can still reproduce the issue.
