Some interaction between the external app helper service and the unknown content type dialog keeps the last inner/outer windows alive for the life of the application. Opening another unknown content type dialog frees the most recent one, so we're probably looking for a cached object somewhere.

Find a file to download, see the open/save dialog pop up, click cancel.

Might be fixed by the patch in bug 420405.
