Hmm, maybe caused by changing my ssh key in bug 433569?
You are using dolske@mozilla.com.

Your LDAP login is jdolske@mozilla.com.  Mercurial uses your LDAP information, so try it with jdolske@mozilla.com.

Hmm. Doh, that's it.

My CVS commits were with dolske%mozilla.com, is there a way to change Hg/LDAP to use dolske@ instead?
There probably is, but we haven't really pursued it.  It will probably get pretty hairy with folks trying all their different aliases etc.  Instead we chose to just stick with the using the base LDAP e-mail as the login credential.

In short, our current implementation doesn't support it.
