bookmarks manager: help cleaning out unused bookmarks by
displaying those site that
(a) the user did not visit for some time
(b) have not changed for a longer time (that requires keeping a
file about changes on the pages)

(a) In Places you can make a query that lists the least-used bookmarks. See http://forums.mozillazine.org/viewtopic.php?t=629560 on how to make your own queries.
(b) is totally different from (a) and so should be in a different bug. Not that it's a bug or a sensible enhancement.. just because a webpage hasn't changed in a long time doesn't mean someone doesn't want to still visit it.

true but in some cases you may want to get rid of bookmarks that link to dead pages.
it really depends on the nature of the site.
if it`s a resource for information then it`s different from a forum, news-page or blog.

a) is WFM (you can show the visit count and last visit date columns in the organizer)
