452057 - Back/forward buttons onto a custom toolbar disappears when upgrading 1.8=>1.9(1.9.1)

The fix on bug 426026 only handles the back and forward buttons for the Navigation and Bookmarks Toolbar. But if users are creating their own additional toolbar both buttons are getting lost and have to be manually placed again onto this toolbar.

Steps:
1. Start Firefox 2.0.0.x and create a fresh profile
2. Open the Customize panel and add a new toolbar
3. Leave the panel open and d&d both buttons onto the new toolbar
4. Close panel and make sure both buttons exists onto the new toolbar
5. Close Firefox and start Firefox 3.0.x (3.1)

After the browser window is opened you can see that both back/forward buttons were removed from the custom toolbar. They should still persist.
