455071 - Fireuploader Error: Database Not Created

When I open Firefox I get a Javascript Application dialog box with a message that says "Fireuploader Error:  Database Not Created"  and there is an "OK" button.  I have to click on the button three times before Firefox will open.  Then, occasionally if I am in a webpage I will get this message again if I am trying to do something (like checkout from an online store).  This started happening when I downloaded the most recent upgrade maybe 5 days to a week ago.

Try to reproduce this in Safe Mode to see if an extension is causing it.

This is a warning created by an extension.
Use the safemode as suggested in comment #1 
- http://kb.mozillazine.org/Safe+mode

and if it works there disable the addons one by one in tools/addon to find the extension that is causing this.

Are you sure that you correctly entered the safemode ?

Yes I am sure.  I used the safemode icon in the programs folder.  I also double checked by running the firefox -safe-mode command manually.  I still get the error message.

and are you sre that Firefox is closed before you tried to open it in the safemode ?
You can also try to disable the addon under tools/addon

Yes, I am sure that Firefox was closed before I opened it in safe mode.  I disabled the one and only addon that I have and I still get the error message in safe mode and in regular mode.  Would you be asking me this same question three times if I were a man?

I didn't know that you are a women, i just don't know why you get this error window which comes from an addon if you run Firefox in the Firefox safemode.

Did this addon install a plugin ?
Can you please check under about:plugins (enter it as URL) if it did.
Can you also try creating a new Firefox profile

I am running Firefox on both my MAC and my PC. I get the exact same error message on my MAC but not my PC. I too have to push the OK button to open Firefox and at online stores, etc. I do not get the message if I use Safari but I much prefer Firefox. Did any of the fixes listed above help or are there any new ideas?
