Overview:  The form controls are rendered in the wrong places and incorrectly 
repeated.

Steps to reproduce:  This is best seen if you have a 4.x & 6 browser handy...
1) Launch the Linux 2000-07-14-21-M17 Commercial build
2) Launch a 4.x build
3) Load the page in the URL on both browsers
4) Note the marked difference

Are you sure?  Bug 45568 wasn't introduced until after the morning's builds on
the 14th.
The bug arrived in linux build ID 2000-071421, which i reported in bug 45560
ckritzer@netscape.com finds it in 2000-071421.
That is why i thought it was a dup. Reopening this one based on comments.

Putting on [nsbeta2-] radar. Not critical to beta2.  Adding "nsbeta3" keyword 
for consideration of a fix for that milestone. Show us some top 100 sites with 
this behavior and renominate.
