User-Agent: Mozilla/5.0 (Windows; U; Windows NT 6.0; en-US; rv:1.9.0.4) Gecko/2008102920 Firefox/3.0.4
Build Identifier: Mozilla/5.0 (Windows; U; Windows NT 6.0; en-US; rv:1.9.0.4) Gecko/2008102920 Firefox/3.0.4

This midi file is local and plays on Windows XP in Opera and Safari
and even Mozilla (not updated in some time), but not in Firefox (3.0.4 - Quicktime is 7.5.5).
It plays in Vista in Opera, but not Safari or Firefox.
On the systems on which it does not play, a Q with a question mark is displayed, not the alternate text.
Note that the web site displays fine - the problem only shows up with local files.

Substituting application/x-mplayer2 for audio/x-midi, the file plays in all browsers (but there
is nothing wmp-specific).
When this page is accessed on a web site using http rather than file protocol,
the midi file plays in all browsers.
This seems to be able to be duplicated with all midi files - I haven't tested other media types.

Reproducible: Always

Steps to Reproduce:
1. Download http://www.dayenu.com/test2/ff3qtbug.htm and dayenu.mid to local file.
2. "Open file" ff3qtbug.htm from file to which you've downloaded it.
3.
Actual Results:
Browser displays Q with question mark, rather than either embedded object or alternate text.

Expected Results:
Should have displayed embedded object, or, at least, the alternate text.

I have confirmed that this problem is limited to FF3; it works as expected in 2.0.0.18. Also, in case I wasn't clear above, it works ok even in FF3 when I access it through http://localhost/ff3qtbug.htm (using XAMPP in Windows), even though accessing same file through file: protocol doesn't work.

I can confirm this to happen with audio/mpeg and audio/x-mpeg as well.
This does not happen on Firefox for Mac, or any browser other than FF3
for Windows.
do you still see this problem with newest version of firefox and quicktime?

I'm afraid the bug persists. Firefox 3.6.8, QuickTime 7.67.75.0. Operating system, if it matters, is Windows XP SP3, with all updates applied.
Problem appears to be resolved in both Firefox 4 and Firefox 3.6.16 with Quicktime 7.69.80.9.
