I found one of the reasons for so many "Content may be out of date" warnings. If you make an edit to a staging copy, and check mark the box to add a "Content may be out of date" warning to translations of the article, that warning gets applied right away. The edit may have been spam and rejected, but the warnings on translations stay.

Will updates to staging still be able to clear the critical translation flag?
