Does the problem go away if you set the form controls to "-moz-appearance: none"? If it does, as I suspect, then the bug is either in the GTK theme or in the GTK native theme painting code.
Component: Layout: Form Controls → Widget: Gtk
QA Contact: layout.form-controls → gtk

Please tell me how to set the controls to such settings. In Epiphany with Webkit engine the entry fields do not disappear. I also opened a bug in Gnome bugzilla, but it has been closed as 'notgnome': http://bugzilla.gnome.org/show_bug.cgi?id=564215
