I've seen a few situations in which shortcuts such as Command-T (for new tab), Command-N for new window, and Command-` (OS X shortcut to cycle through windows of the same application) stop working when there are several tabs open.

Sometimes all of these shortcuts work fine; I haven't yet figured out the exact set of circumstances that trigger the bug, but it seems to be when there are lots of different types of elements present that can change the current focus, such as flash video players, web forms, etc..

Reproducible: Sometimes

Steps to Reproduce:
1. open a couple firefox windows, and use firefox for a while
2. In one of the windows, open several tabs, and in those tabs, open several different web pages, preferably with different types of elements such as flash videos, forms, etc., etc.
3. try using the shortcuts I mentioned in the bug report; try changing the focus such as clicking the mouse somewhere on the page with no element, or somewhere in a web form so a specific field is highlighted, or clicking on flash video player buttons, etc.

My current OS X version is 10.5.5 / build 9F33
First try if you can also reproduce it in safe-mode, because bugs in extensions can cause a lot of trouble: http://support.mozilla.com/en-US/kb/Safe+Mode
