Details

The confusion around this is complete; they are treated as synonyms and no code compiles with only of of them defined. They are not used consistently; some chunks of code use both. IMO we should standardize on DEBUG (or VMCFG_DEBUG?), not _DEBUG, and for backward compatibility the headers might define DEBUG if _DEBUG is defined.
VMPI.h makes sure both are defined if either is defined (since a while back). Good enough.
