Page added to address licensing -- user will choose one of two choices in order to continue.  This page should be present after register/signin and be in the flow for each persona a person submits (vs. just one per username).

Talked with Ryan and agree that it makes sense to combine the "create" step and to present the license option on the same page.  I've attached modified flow to reflect this, as well as added in the user agreement page (which had fallen off the previous version).
