This is a huge pain -- I manage multiple earthlink-hosted sites for my clients 
and the following three fields are required:

Domain: This identifies which earthlink account is to be used.
Username: This is invariably "admin" for all accounts.
Password: As you would expect, this is invariably a strong, hard-to-remember password.

The way this login page works, the desired behavior with password manager would
be as follows:

1. Load sign-in page
2. Select previously-entered domain name from dropdown(like the username field
on gmail, for example)
3. Password manager should then enter the username and the correct password
based on the "Domain" entry.

Additionally, password manager is getting very confused about which field is
which on this page. This is illustrated by the following password manager 
screenshot:

Note that the domain is stored in the username field, and the username is
stored in the password field.

This used to work fine(as described above) in Firefox 2.
