A draft of the new design and questions is at http://surveys.mozilla.org/?id=12.  We will be using surveygizmo to create the new Fx uninstall survey and it will be hosted on a new mozilla.com page (URL TBD).

Implementing the iframe or js solution for the new survey on mozilla.com trunk will be a separate bug.

Just a heads up that I should have the survey ready later today.  At that point, Fred can get started on the embedding and URL redirects.

The Uninstall Survey is ready to implement!

This bug was assigned to me because it was merely for me to finalize the actual survey questions and create the survey in surveygizmo.  Bugs 489698 and 489696 (along with comment #2 here) are for you to do your implementation magic.
