If "Activity Indicator" is added to the menu bar, unchecking "Menu Bar" does not hide the menu bar.  And furthermore, if a separator or flexible space is added to the menu bar, each time "Menu Bar" is toggled, they are duplicated.

This is a regression from Bug 456535


Reproducible: Always

Steps to Reproduce:
PART I
1. Right-click the menu bar and choose Customize...
2. Drag and drop the "Activity Indicator" to the right of the "Help" menu
3. Right-click the toolbar and uncheck "Menu Bar"
4. Notice that the menu bar does not get hidden.

PART II
5. Right-click the menu bar and choose Customize...
6. Remove "Activity Indicator" from the menu bar
7. Drag and drop "Separator" and "Flexible Space" to the right of the "Help" menu
8. Right-click and uncheck "Menu Bar"
9. Right-click and enable "Menu Bar"
10. Notice that both the Separator and Flexible Space have been duplicated.  You may have to choose Customize... to see the Flexible Space duplication.
11. Keep toggling "Menu Bar" and notice that Separator and Flexible Space duplication keeps occurring.
