According to numerous reports on the support forums, the AVG toolbar seems to be preventing new tabs from opening when using Ctrl-T or double clicking on the tab bar. This may be related to putting Yahoo Search on new tabs as some reports suggest that disabling that behavior works around this issue. See linked forum threads for first-hand reports.
Apparently everything BUT File > New Tab works.
Summary: AVG toolbar adds Yahoo search to new tabs and blocks using Ctrl-T or double clicking on tab bar from working → AVG toolbar adds Yahoo search to new tabs and new tabs from opening

SUMO thread: https://support.mozilla.com/tiki-view_forum_thread.php?comments_parentId=366264&forumId=1 Adding AVG to this thread, as well. Petr, can you provide a contact for discussion on the design change on new tabs that the addon introduces, and to look into the issue outlined in the thread above? Feel free to email me directly as well, and I can update this ticket as needed.

I've had exactly this problem. By disabling the "AVG security toolbar" add-on (2.506.014.001 on my machine) the problem goes away.
Confirming and adding as a common issue based on having even more forum reports.

I'm not sure what the point of this bug is. As filed it's probably invalid and should be reported to AVG. If that AVG Toolbar version needs to be in a blacklist, then I think this bug is in the wrong component.

We're now tracking such bugs. This doesn't mean it's something we can fix, merely something we hope to be able to point vendors to so they can investigate. This is an automated message.

Closing old bugs in the Plugins component. We aren't going to track issues in 3rd-party plugins in the Mozilla bug tracker. In addition, support for NPAPI plugins will be removed at the end of this year; for more details see the post at https://blog.mozilla.org/futurereleases/2015/10/08/npapi-plugins-in-firefox/ If there is a serious bug in Firefox, it needs to be filed in the "Core" product, "Plug-Ins" component.
