According to numerous reports on the support forums, the AVG toolbar seems to be preventing new tabs from opening when using Ctrl-T or double clicking on the tab bar.  This may be related to putting Yahoo Search on new tabs as some reports suggest that disabling that behavior works around this issue.

I've had exactly this problem. By disabling the "AVG security toolbar" add-on (2.506.014.001 on my machine) the problem goes away.
Confirming and adding as a common issue based on having even more forum reports.

I cant get a new tab no matter what I do
I'm not sure what the point of this bug is. As filed it's probably invalid and should be reported to AVG. If that AVG Toolbar version needs to be in a blacklist, then I think this bug is in the wrong component.
https://support.mozilla.com/tiki-view_forum_thread.php?locale=en-US&comments_parentId=414062&forumId=1 is the only report from this month... may just be fading naturally.
