User-Agent: Mozilla/5.0 (Windows; U; Windows NT 6.0; en-US; rv:1.9.0.11) Gecko/2009060215 Firefox/3.0.11 (.NET CLR 3.5.30729)
Build Identifier: Mozilla/5.0 (Windows; U; Windows NT 6.0; en-US; rv:1.9.0.11) Gecko/2009060215 Firefox/3.0.11 (.NET CLR 3.5.30729)
I was browsing within 3 Firefox windows consisting of about 150 tabs (using slightly over 1,100,000K of memory) when an error message dialog box appeared stating (paraphrased from memory, I forgot to screencap it):
"The application has sent an unusual request to be terminated."
The error message window only had an "OK" button, so I clicked it, which closed the Firefox process. I reopened Firefox and selected "Restore Previous Session" when prompted. However, instead of restoring my session properly, it only restored the first Firefox window and its tabs. Furthermore, it disabled the functionality of most (but not all) of my add-ons, but they were not disabled in the Add-ons manager.
I wish I could provide more details, but this is how it happened as I remember it. Though, if there is relevant crash information logged somewhere on my computer, I may be able to provide it with some instruction.
Reproducible: Didn't try
Actual Results:
Firefox crashed. On a Session Restore it failed to properly restore my session and most of my add-ons lost functionality despite not being disabled.
Expected Results:
Optimistically, Firefox shouldn't have crashed. However, in the event of a crash, it should have performed a Session Restore properly and the add-ons losing functionality is perplexing.
Add-ons used:
Adobe DLM 1.5.2.35
Ctrl-Tab 0.20
Download Statusbar 0.9.6.5
DownloadHelper 4.4.1
DownThemAll! 1.1.3
Find In Tabs 0.4.0.3
Japanese-English Dictionary for rikaichan 1.10
Multiple Tab Handler 0.3.2009051501
Next Tab 0.2.2
Perapera-kun 1.0
PONG! 2.3
TabCounter 1.0.3
Without further information nothing at all can be done about this. If you can reproduce with all extensions disabled, can provide a crash log and whatever errors show up in the error console then please reopen this bug. As it stands, some of those extensions could have caused this issue.

Since your replay, I have crashed Firefox twice while attempting to reproduce the initial crash. I disabled or uninstalled all extensions. Both times the Crash Reporter "Failed to send the report properly" (or "There was a problem sending the report", I'm not sure exactly what it stated), which is, in itself, a problem. There is nothing in the Error Console upon reopening Firefox because it has restarted, therefore clearing itself. I have located the directory for crash report files, though I do not know which ones are useful for your purposes. I can attach the .EXTRA, .DMP, or other files if you let me know which would be helpful to you.
