I used to use version 0.9, and since upgrading to 1.0 in May (first public 1.0 release), and now 1.0 b2, I've noticed that Prism doesn't always create Start Menu shortcuts via the 'Web Apps' folder listed in 'All Programs'. 

I've tried creating new apps with several varying sets of options selected, even no options selected (except that each time, the start menu shortcut option was always selected). Results of my testing seem to point to the issue occurring when both 'Desktop' and 'Start Menu' shortcut options are selected. 

When 'Desktop' is not selected, and 'Start Menu' is selected, it seems I can use any other options as well, such as 'Quick Launch' or any of the show/enable/display options above, without having any issues. 

So it seems 'Desktop' shortcut creation prevents 'Start Menu' shortcut creation. It also seems to inhibit 'Quick Launch' shortcut creation.

Reproducible: Always

Steps to Reproduce:
Try creating an app and selecting all three shortcut options. The non-desktop shortcuts are never created. 
Actual Results:  
Only the desktop shortcuts are created.

Expected Results:  
All three (or two, if desktop + one other type selected) shortcuts should have been created.
