506913 - Re-organize account registration page and add fields

After a bit of thinking, and depending on complexity of the modifications, I'd like to suggest some tweaks to the current registration page as follows:

Personal Details:

- Rename "Personal Details" to "Account Details"
- Split out full name into "First Name" and "Last Name" fields
- Move "Organizational Details" into the "Account Details" section
- Add "Website" field
- Add "This is a personal account" checkbox to identify non-organizational accounts (be nice if it'll grey out org details when checked, but that's a nice-to-have)
- Change address from free-form to:
   - Street Address 1
   - Street Address 2
   - City
   - State/Province
   - Zip/Postal Code
   - Country

All fields are mandatory with the exception of the Fax number and Website. The Org info should only be optional if the personal account flag is checked.

Let me know if these changes make sense, and whether they can be modified in-scope.
