prefs block pop-up windows checked, pop-ups on pages still open (pop under, in new windows) and notification bar states pop-ups were prevented (in this case 2 windows).

Can you give an example ? Some pop-ups are actually allowed, because they're used by websites (after a mouse click or keypress). There are a few unwanted popups (advertisements, etc ...) that take advantage of this loophole, but they're rare now.

increasing frequency of pop-ups opening new windows even though Firefox has given a "pop-up blocked" message.
