Seen while running Mozilla/5.0 (Macintosh; U; Intel Mac OS X 10.6; en-US; rv:1.9.2a2pre) Gecko/20090916 Namoroka/3.6a2pre

So you saw this, because you are an admin and you saw a unpublished comment (see the "unpublished"text beside this comment :) , because this user didn't signed up on qmo. Its a spam protection 

Maybe the bug is here that the reply button is shown for unpublished entries.

Yes, I think the bug is that the reply button should be shown for unpublished entries. I file it and point back to this bug.
