Dismissing some dialogs results in a 1 inch square window that has garbage
characters like the "1/2" symbol in the title. This window must be killed using
the window manager since it has no chrome or content.
The dialog I know for sure did this was the "repost form data" dialog when I
cancelled it.

nominating, since the window must be manually dismissed using the window manager
close button and looks really really bad.
