In Fennec we implement our own prompt service, but sometimes it is not registered after Mozilla's.  Even throwing Cr.NS_ERROR_FACTORY_REGISTER_AGAIN does not do the trick because for some reason Moz components are registering twice, the second time after Fennec's components are registered.

Benjamin, have you seen this on a device ? I've never seen it on a desktop/linux build.

I've only seen it so far on Mac debug builds.

I've seen symptoms that appear to match this on a Linux desktop build, and Aakash saw them on a 1.9.2 build for Maemo.
