When a compatibility option is selected, the report dialog opens and its contents depends on whether it's a pass or fail report.

Pass reports only have basic info that can't be edited, as shown in the mockup.

Fail reports have a textarea for user input, a checkbox for whether to include other installed add-ons, and an option to disable the offending add-on.

When the report is submitted, the fields should be sent to the API for recording, as described in bug 521307.

I am not really sure, I just meant to give it another pass. Is there anything out of place that you notice.

I think for first release it is fine.

Essentially nothing. Any issues in that dialog should be reported as new bugs, and I don't know any at this time.
