is there a way to find out which versions of quicktime are invovled? if there is and they are old we should consider blocking.

I have some quicktime contacts
A testcase that always crashes with Firefox 4 and Quicktime 7.6.9 on Windows: https://zzxc.net/sumo/embedwav

This is quite common in crash automation with the same top frame but a variety of different lower frames but they all have in common: QTAudioDeviceContextCreate:AudioContextInitialize failed just before the crash. This is with QuickTime 7.7.1.0 on Windows XP and Windows 7 and all branches Beta, Aurora, Nightly. Kev, can we get someone at Apple to look into this?
Summary: Crash @ QuickTimeWebHelper.qtx → Crash @ QuickTimeWebHelper.qtx@0xec19 due to QTAudioDeviceContextCreate:AudioContextInitialize failed
we finally have an update to qt: http://support.apple.com/kb/HT5261 (hattip tomcat). I'll retest as soon as I can update the machines.
Continues with QuickTime 7.7.2.0 @ QuickTimeWebHelper.qtx@0xedbf A new twist is the addition on all branches of a relatively small number of ABORT: CRT ERROR R6010 (if you have the patch to report them) associated with the fatal assertion Assertion failed: ptr_ != NULL, file c:\work\mozilla\builds\aurora\mozilla\ipc\chromium\src\base/scoped_ptr.h, line 93 though these aren't reliably reproducible.

and the saga continues with qt 7.7.6 bp-17f7d-f9f7-4067-88b2-11fd42150514 we don't test with this automatically any more since qt is click to play. We can just mark this wontfix since a) it isn't our bug and b) they won't fix it.
OS: Windows XP → Windows
I guess that's probably best. At this time, I do not think that the amount of people watching QT video on websites is significant any more - or do you disagree?
wontfix it is then.
