Which addons do you have installed? Please run Firefox in Safe Mode to check if an addon is causing this problem. See http://support.mozilla.com/kb/Safe+Mode.
URLs -- web2.westlaw.com & print.westlaw.com. I have several addons installed. However, I opened Firefox in Safe Mode and added the two sites above to the popup exception list (Tools, Options, Content, Block pop-up windows checked, Exceptions, Address of Website keyed in, Allow, Close, OK). I closed Firefox Safe Mode, reopened Firefox in Safe Mode and the two sites were no longer listed in the "Allowed Sites" list.
I should add that, the same steps produce the same result in Firefox 3.5.5 using Safe Mode. Also, prior to Firefox 3.5, this feature did work with these URLs.

Are you deleting any items of your recent history when shutting down Firefox? Please check under privacy inside the options. Does a fresh profile shows the same problem for you? See http://support.mozilla.com/en-US/kb/Managing+profiles

Wow! This bug is more than 12 years old! I came to report the same!
The "Clear History..."-feature, specifically the "[√] Site Preferences"-option under "Data" in the "Clear History..."-window in Firefox removes the "Block Pop-Up Windows" exceptions. This is unexpected. At least for me, and possibly others as well.
I have Firefox set to "Use custom settings for history" and "[√] Clear history when firefox closes ..", with the "[√] Site Preferences" ticked.
When rubber-ducking this "bug" at this moment, i realise that it could be argued that an exception to block pop-ups is the same as an exception to allow microphone usage or the likes, but it feels like pop-up block exceptions are more of a browser-feature-thing than a Site Preference per se.
Perhaps make this an 'Improvement'-type bug, and make clear what "Site Preferences" actually are
