I added a third add-on to my collection and then restarted Fennec and it didn't detect it. Is this due to caching?
Yes. 

Well, more specifically, it doesn't fetch the collection every startup, only once per update interval (default 8 hours). So if you wait for 8 hours and restart, it will appear :)

I'm not sure the correct behaviour for this. Fetching every startup would be excessive, but maybe we could reduce the default update interval for Fennec.
Hmm. I had wanted it to be every startup, but if it's a bad perf hit, I suppose adding the button for manually syncing will be sufficient.
I added a manual sync button and stopped any auto install on startup.
