The bookmarks toolbar appears to be empty, and I am unable to add items to the bookmark bar. also the star button to add bookmarks seems disabled and the back button doesn't work sometimes.

You should try to reproduce this with a new profile or in safe mode[1]. You may have a corrupt places database as well.

I have the same problem. My toolbar comes up when I load up in safe mode. How can I repair my profile?
