We've seen a few reports of this lately:

On checking the status of my extension (approved 3.5 hours ago) the following message was on the status page.

Add-on Status: In Sandbox; Pending Review
Your add-on is pending. This shouldn't have happened. Please e-mail amo-editors@mozilla.org with your add-on ID and state this error.

I'm assuming that the add-on ID is the name: Toggle Proxy

We should make sure the editor review page is working properly in all cases where it approves an add-on. Add-ons should never be marked as STATUS_PENDING (2). I previously assumed these problems were from someone manually marking them that status through the admin panel, but this sounds like it was automated.
I changed the status of this particular add-on by hand, and I guess I did it wrong. The problem was that the author wasn't able to upload an update and nominate it to the public. That is something I've seen frequently.
