532812 - The form for reporting trademark violations doesn't work correctly

The email that results from submitting the form located here: http://www.mozilla.com/en-US/legal/fraud-report/index.html doesn't capture the selections made from the drop down lists under Category, Product, and Trademark. Those fields are just blank in the resulting email. See the attachment. Thanks.

This is a critical tool for legal. We really should get this fixed ASAP.  If Bugzilla was failing like this, everything would come to a screeching stop. This is an equivalent tool for our legal team and they shouldn't have to wait weeks or months for a fix. 

Is there anything more that WebDev needs from the reporters to get this fixed?

Asa, you're 100% right, my apologies.

I couldn't ever find an error.  I added a way to send a test email to myself, and that email works fine for me.

Try it out, if it's still broken for you, reopen this bug.

Thanks Alex. From the test emails I've seen it looks like it's working now. I asked someone from outside Mozilla to submit a test form to confirm this. Will let you know if we see any other problems. Also, we probably want to modify the text of the reporting form page. Should I open a separate bug for that?
