If the "Save a file" dialog box has the misfortune to appear just before a webpage triggers a "cookie settings" dialog box, Firefox will get stuck. Everything still works (downloads continue in the background, etc.), but the Download box's buttons will be inactivated (because the Cookie Settings box appeared)... and the cookie settings box itself will be inaccessible because there's a Download box in front of it! Instant catch-22-- and to get out of it, the only solution is to hard-quit Firefox.

This can happen on some websites that trigger pop-ups when you click on a download link.

The bug appears to be gone in Firefox 4 (all versions after Beta 3).
