First, we need an option to display radio buttons with freeform input. Next, we need an option to display an additional explanation at the top of the survey page, or beneath a particular question in smaller type. Next, the survey answers appear to display incorrectly in some cases when the page is reloaded and "change answers" is clicked - an index=0 check box gets checked even if the user never checked it. This happens even though the survey answers stored in preferences are correct. Finally, if you click twice on "change answers" without reloading the page, you'll end up seeing two copies of the form on the same page, which looks very confusing.

Radio buttons now displayed with freeform input, explanations added, and bugs fixed in this patch contributed by Appcoast: http://hg.mozilla.org/labs/testpilot/rev/af06746ba39b Still to do: Display checkbox next to freeform input; implement FREE_ENTRY type which is just the freeform input with no checkboxes or radio buttons; and finally, need to add versioning for surveys like we have for studies.
