I believe your question is, "If my domain has 'moz' in it, do I still need to fill out the 'Domain Name License' form (http://www.mozilla.org/foundation/trademarks/domain-name-license.pdf)?" If that's correct, CC'ing gerv and hja to answer.

Helo i thought you only did a domain forum if mozilla was in that only and if i use moz-host then i need to do a forum

It depends on the specific domain name. Please complete and submit the application linked in Reed's comment for our review. You can send it as an email attachment to trademarks@mozilla.com. We'll let you know if permission isn't required. If our permission is needed, we'll let you know if we grant permission or not based on the application.

as I got told if it only has mozilla in that then it does need to get a domain lience

Per our trademark policy, available at http://www.mozilla.org/foundation/trademarks/policy.html, "If you want to include all OR PART OF a Mozilla trademark in a domain name, you have to receive written permission from Mozilla." Please submit the application located at http://www.mozilla.org/foundation/trademarks/domain-name-license.pdf to request permission.

I agree it would be helpful if the domain name application form could be completed and submitted online, but we haven't gotten around to doing that yet.

He's referring to this paragraph in the agreement:
5. Licensee Representation as to Age. Licensee, if an individual, represents that
Licensee is eighteen (18) years of age or older as of the date that Licensee
signs the Application.

on facebook and on myspace if I use the name firefox do I need to do the forum also
and I spoke to the marketing team about that and they know I am setting it up

As long as you don't use Firefox in a way that implies your are affiliated with or sponsored by Mozilla, you can do whatever marketing has approved.
