This is the boilerplate others are to use to make their contributions.  But the
Original Developer and the Original Code are already filled in with Netscape
security code info.  They should be blank.

This is true of all the MPL dual files.

It's also true for the NPL files, but maybe this is OK (or even correct) because
we don't expect anyone else to contribute anything under the NPL.

We don't expect anyone to use the dual or NPL boilerplates any more; all new code uses the tri-licence boilerplate.
