please, open your Error console, and evan this code: Components.utils.import("resource://gre/modules/PlacesDBUtils.jsm");PlacesDBUtils.checkAndFixDatabase(); after a minute you should get a log in the console, paste it here. Which extensions are you using? Have you tried adding bookmarks in Safe Mode (http://support.mozilla.com/kb/Safe+Mode)? This temporarly disables your add-ons. Do you use external software to clean up Firefox history like CrapCleaner or similar?

does this happen for all bookmarks or only certain bookmarks?
Seems to happen to all bookmarks that i use in a session but not every time. Example : i have facebook bookmarked with the blue f as an image I open a new tab and click on the button to go to facebook Using the same tab i go to google Then from there i go to google news... Then a few seconds latter it happens , my facebook bookmark has the googlenews image instead of the facebook image.

I have this problem in Mac OS. Mozilla/5.0 (Macintosh; U; Intel Mac OS X 10.6; en-US; rv:1.9.3a5pre) Gecko/20100413 Minefield/3.7a5pre I had 2 recent bookmarks that changed address by itself. At first I though it was updated Weave 1.2 (and later 1.2.1) but after disabling it the problem remains, except the address is what I had bookmarked and now only the favicons are being changed to sites I have recently visited. Answering question in comment #3, it is only happening to 2 specific bookmarks (the most recent I've added).
I can confirm the problem, it's the same here. For example I bookmarked my own server which always provides the same favicon. Sometimes this icon (randomly) changes to another (recently visited) site's icon. Then I try to open my server's homepage in order to restore the correct icon, but without success (even tried to clear the cache and made a hard reload). Now I also tried the function as suggested in comment #1 and after opening my server's page the correct icon was restored. However, this little success just lasted for 2 minutes because now my server's bookmark has again a wrong icon - the same icon as this bugzilla page. I'm calling this a dupe because that's the only known reason we know. Please reopen the bug only if you can reproduce in Firefox 4.0.
