Particularly important is the build ID you tested with because I suspect from
this minimal report that you may be experiencing problems already reported and
recently fixed in bug 53568

Yes it may be a duplicate of bug #52568 since the street and city input fields
are grey.

To reproduce simply visit the URL in this build.

Navin Gupta can you brose this site with today's build and if it is corrected
Verify this a duplicate of 53568.  It looks good tome in 092908 linux mozilla
trunk build on RedHat6.2
