Hmmm ... yesterday I saw something similar in <http://www.alcatel-lucent.com> (there should be a submit button in the popup). I noticed that I could see the button again if I removed the type="submit" attribute.

I haven't had time to investigate what was happening, or in which version.
Maybe a regression from bug 166235, fixed by patch in bug 558726?
I have verified it's fixed by bug 558726 in a local build,
unfortunately the tree is closed though :(
Depends on: 558726
Should be fixed now, please verify with tomorrow's nightly build.
