thunderbird patch

This is *quick and dirty*, but shows what issues I am hitting getting Contacts running in TB.  This includes the gmail patch I posted to bug 557882.

I've also created an importer to pull in nsIABCards from the tb address book, right now hard coded to the collected addresses book.

updated patch against a pull from this morning, also fixes links and prevents a div from interfering with working links

added a menu item to tb's message header to view people in Contacts (screenshots at http://shane.caraveo.com)

closing this, tb integration in contacts is long done.
