It usually has to do with flash. every time the error report box comes up and an option to send a report is taken (through exit or restart), the error report fails to go through... EVERY TIME! Reproducible: Always
The crash reporter will not send if you are behind a proxy or there is a firewall that is blocking the connection. http://support.mozilla.com/en-US/kb/Firewalls looking for any instance of your firewall blocking crashreporter.exe and delete the block rule.
