563928 - Merge the three "Account Request" components into "Repository Account Requests"

Under the new commit access policy, account requests are no longer given on a per-SCM basis, but at a particular level. Therefore, it seems unnecessary to have three components for account requests when one would do.

"SCM Account Requests"? Or a better name? What do you think?

Many newbies won't know what "SCM" stands for. Reed had some ideas about this a while back as well. I am in favor for anything that will make it clear what the person is asking for, and since the bug summary often does not contain that information I am not sure what is the best path.

I'd suggest renaming the CVS one and then merging the other two in, it has the largest number of bugs, so doing it this way would result in the least amount of bugmail.
