Saw one comment in a forum that could help to identify the source of this .dll as WebcamMax.
"Internet Explorer has stopped working. A problem caused the program to stop working correctly. Windows will close the program and notify you if a solution is available."

I suspect this is a WebCamMax problem, but I thought I would share it here as well. Works fine in Firefox.
and i got it to work by closeing Webcam max.
Attempting to start the broadcast again and it stop crashing. and loaded up.
I changed the cam to The webcam max fast one and it asked for permission and loaded up webcam max its self and seem fine after that. no crashing.
probably need to install from http://www.webcammax.com/ to test this out or find contact info

jst mentioned that flash might load this activeX control.
i don't think .ax stands for activex, it's basically an audio-video filter for mpeg/directshow.
Roughly flash accesses the user's camera, to do that, it uses directshow which exposes the camera by loaded the registered directshow library which actually knows how to talk to the camera (and also probably has its own ui to lets the user control various things).

This software also has the ability to use the ActiveX Flash Player plugin. For some reason you can use flash content as a filter or effect. Its possible that some custom filters are causing this crash.

Closing old bugs in the Plugins component. We aren't going to track issues in 3rd-party plugins in the Mozilla bug tracker. In addition, support for NPAPI plugins will be removed at the end of this year; for more details see the post at https://blog.mozilla.org/futurereleases/2015/10/08/npapi-plugins-in-firefox/
If there is a serious bug in Firefox, it needs to be filed in the "Core" product, "Plug-Ins" component.
