57181 - form POST submission broken in XUL, working in XML, HTML

In the course of working on a form to access the phonebook, ran into problems
making the form work in XUL.  I'll attach all three versions of the form (.xul,
.xml, .html).  Form is POST and works in HTML and XML.  Returns a page reporting
a server error when submitted from XUL.

The forms are attached above.  You can run them directly from the links --- type
a name to look up in phonebook and hit 'enter'.  View source to compare their
implementations (fairly identical except for namespacing).

given an a= to this patch already.  Ben just needs an r=.

This has since been fixed, I think. I chcked in the patch to remove the hidden 
form....
