572908 - Need to make it clear that feedback will be public

We don't want people entering email addresses or phone numbers and then being surprised that the information is public.

* "Feedback will be in a public database"
* "Feedback will be read by Mozilla employees and volunteers"
* A link "Read feedback submitted by other people", subtly implying that new feedback will also be readable by a large number of people.

We didn't do this for the report a broken website feature. On top of that, we show the link to the public dashboard after they submit their data. It's something that will be nice to have if it makes sense with our wording, but its not a top priority.

There's one possible alternative here: 

Change "Enter your feedback here" to "Enter your feedback to our public
dashboard here" in the textfield of the submission pages.

Note though that since the form field is focused on page load, you might not see that unless you click out of the empty field.

Let's just unfocus the field and change the text in the textfield to "Enter your feedback to our public dashboard here".

I really don't like that solution.  For most users it's an extra click.  For blind users it's invisible.

Some notes:
- The orange box should be the same coloring and have the warning image seen here: http://blog.mozilla.com/security/files/2010/06/warning-screenshot.png
- The text should specifically read "Please ensure that there is no personally identifiable information here." 
- The submit buttons should be moved to the right, so l-to-r readers see it before submitting.
