On an unrelated note, update to service pack 2. Just service pack 1 is no longer supported.
Then, try running in safe mode, http://support.mozilla.com/en-US/kb/Safe+mode. Will it start up then?
Version: unspecified → 3.6 Branch
Same no-GUI hang happens for me with Vista SP2 with Firefox 3.6.8.
Tried clean uninstall, profile wipeout, regentry clean,
then reinstall: still same problem.
Launching in safe mode: same problem: process is there, but no GUI.
Looks like thread deadlock to me.
I've attached 2 jpgs showing 1) thread overview and 2) stack of most active thread during hang.
Others have this same problem: see related discussion at
http://support.mozilla.com/en-US/forum/1/704048?forumId=1&comments_threshold=0&comments_parentId=704048&comments_offset=20&comments_per_page=20&thread_style=commentStyle_plain
Reverting to 3.5.7 : Firefox works again fine for Vista SP2.
(other reports say that 3.6.3 is fine, but 3.6.4 is broken)
Should add that it's not an extension problem: at clean reinstall,
there were no add-ons/extensions but still no GUI appeared.

Just because you reinstall it does not mean that you don't have extensions interferring. Unless you deleted the profile from the old install.
After I uninstalled via the Control Panel, I removed directories
c:\Program Files\Mozilla Firefox\
%USERPROFILE%\AppData\Local\Mozilla\
%USERPROFILE%\AppData\Roaming\Mozilla\
which would have deleted the profile.
exactly same problem here...
I did a full, clean re-install, rollback to 3.x, reinstalled graphic drivers
and DirectX, none of this helped...
Firefox starts (firefox.exe process in task manager), but no visible windows...
