After setting up the tool bar (adding/removing icons), the icon "new tab" (eq. Ctrl-T) has disappeared and no options to put it back :-(

Reproducible: Always

Steps to Reproduce:
1.Install Firefox4
2.Update the tool bar
3.Icon "new tab" is gone
Actual Results:  
Icone "New tab" is gone (from the toolbar AND the list of available icons)
This is by Design.
You can drag the "New Tab" Button off the Tab Toolbar and drop it on the Navigation Toolbar.

Please note that this bug has NOT disappeared with Beta2 (stil not "new tab" button available to put in the toolbar)

Yes, this bug has never been considered by any members of your team, but it is very real and current.

I think nobody has got a "new tab" button in the Customize Toolbar anymore. Am I wrong? I just updgraded from 3.6 and noticed it. I created a new profile and still doesn't appear. Why "New Tab" has been removed from the available buttons in the customize window?
To be clear: many people want two buttons as they always had, one in the tab bar (and we have it) and another in the toolbar beside back/forward buttons. By now, if you have a "+" icon in the tab bar, you can't have another in the toolbar. That's a regression in the UI.

Please notice that there are many discussions about this on mozillazine and SUMO. Please fix.
Windows Vista, with new window opened or initial start of blank FIREFOX browsing session, + "PLUS" is missing altogether, have to use file menu to open new tab, however, once enough tabs are open to fill tab bar, icon appears as does the tab scroll arrow and tab list down arrow, 
most annoying..please fix, also new 20.0.1 tab and main bar are the same colour, without a theme change, I cant seem to alter, after all this time, I would have hoped that a simple colour palette option would have been written in to change basic colour options of how FIREFOX looks, a simple basic assignment of colours to different parts of FIREFOX, ie tab background colours, text colour and size, main menu bar background, a simple pref look for FIREFOX look without having to resort to themes that just dont work for majority of users, most stick with basic, but basic no longer works, I know this portion may belong elsewhere, but a quick vent may spark someone to put it where it belongs, as this is my first post
