The form should take care of logistics surrounding an employee's departure from the company. Form should include: - reason for leaving - manager (from LDAP on date of filing) - date of notice given - last day of work - equipment to be returned (IT inventory can tell us this) - send notification to current manager, erica and karen Seems like bugs that needs to be filed are similar to the newhire form: - bug for HR - bug for IT/asset management stuff The manager should be cc'd on the above bugs if possible.
Ok, so here's what we need to do: - set up a new component in mozilla.com to cover the tracking bug for employees leaving - any time someone uses the form that bug gets filed with all the details - subsequent bugs are filed at dependencies/blockers for the tracking bug

The current new hire form just uses the existing Mozilla Corporation :: Facilities Management component. Do you want to just continue to use it but just have a different summary for termination notices, or do you actually want a completely separate component?
I think it should be a separate component so they can track people leaving in a queue.
We should be able to build off of this code base to create the form we need. From there, HR can use bugzilla for their queue/checklist. http://svn.mozilla.org/projects/webtools/workermgmt/trunk/application/
