This bug's being caused by the Ubuntu Firefox extension (which is incompatible). I'd close it as invalid but I'm not sure why one broken extension is able to affect another.

Summary: Missing toolbar buttons from extension → Missing toolbar buttons from extension with ubuntu firefox extension enabled
Legacy extensions are no longer valid in Firefox 57, closing.
