When I first dragged the sync toolbar button onto my toolbar, I immediately found a similarity between that one and the reload button. Especially as Internet Explorer also uses two opposing circular arrows to indicate the reload button.

I'm not sure whether there's a way out of this, I just think that it would possibly confuse future users.
Is this bug still valid in recent versions of firefox?

Yes, the icons can still be confused especially if you customize put the Sync icon on the URL/search toolbar.
That said, unless you want to change the Sync logo altogether, I'd be inclined to won't fix this.

Besides, the Sync toolbar button doesn't have a tooltip. Maybe we should add one as first measure.

yes, a tooltip would be helpful.
We don't have a toolbar button now. We have an add-on bar button, but that's at the bottom of the screen and really hard to confuse.
