We attempt to extend first letter to cover the first grapheme cluster, but the results aren't always correct for Indic text. The document in the URL shows several examples, which need to be extracted into testcases. I'm attaching a testcase for one example where the Unicode codepoints are specified (plus another case which we do get right).
Note that the shaping *is* correct in the second testcase.
